Hypoperfusion rapidly leads to organ dysfunction in severe sepsis. This systemic response to inflammation is an emergency. Immediate resuscitation should not be delayed by investigations as resuscitation can prevent further organ damage. A, B, C is more urgent than an exact diagnosis in the first instance. Diagnosis and the hunt for a source of infection can safely follow later. From the pathophysiology discussed so far, a methodical approach to resuscitation in severe sepsis is outlined in Box 7.2.
