From the information provided by the most recent trial,48 interferon alfa-2b is the most effective adjuvant treatment now available, with a significantly improved prolongation in relapse-free survival and overall survival compared with vaccine therapy in patients with resected high-risk melanoma. The rate of severe or very severe side-effects is high. Further studies are now underway combining interferon alfa-2b with other peptide vaccines as well as with polychemotherapy plus interleukin-2. Low-dose interferon alfa-2b may also have a disease-modifying effect, but as yet no benefit on overall survival has been shown.
