Salicylic acid is the most commonly used keratolytic agent and is often advocated for removing psoriatic scale. It is considered beneficial not only because it reduces scaling and skin shedding but also because it thereby removes a barrier to the penetration of more active compounds into the skin. Concentrations between 2% and 10% in an ointment base are usually dispensed. Salicylic acid is often used in combination with coal tar or corticosteroids. There is little information from RCTs on the efficacy of salicylic acid monotherapy as a descaling agent.
