The ambulatory treatment of culture-proven S. viridans endocarditis has been advocated.22 Patients treated with intramuscular injections of ceftriaxone, 2 g daily for 4 weeks, did well. This finding should not affect emergency medicine practice, however, since cultures are not usually available until some time after admission to the hospital. For patients for whom blood cultures are drawn and who are then sent home, a referral should be made to a cardiologist for evaluation and follow-up.
