Diagnosis

Periorbital and orbital cellulitis are distinguished from noninfectious disorders on the basis of the clinical findings and the WBC count. Leukocytosis occurs frequently with cellulitis, more often with bacteremic preseptal infections. A blood culture is often positive.

CT is performed when orbital involvement is likely. An inflammatory mass is easily demonstrated when present using this modality. Differential Diagnosis

As for cellulitis in other regions, allergic and traumatic causes for edema must be considered. Additionally, tumors and metabolic disease may cause swelling, discoloration, and/or proptosis. Thyrotoxicosis usually occurs in adolescents. The most likely tumor is metastatic neuroblastoma. Pseudotumor occurs rarely.
