A lateral approach to the elbow is used. Place the elbow in 90 degree flexion, resting on a table, with the hand prone. Locate the radial head, lateral epicondyle of the distal humerus, and the lateral aspect of the olecranon tip. These three landmarks form the anconeus triangle. The center of this triangle is the site for needle entry into the skin. The needle should be directed medial and perpendicular to the radius ( Fig . .278-2).
