In intracerebral and SAH, a cerebral blood vessel ruptures, with a dramatic increase in intracranial pressure and a decrease in global cerebral perfusion that can last for minutes. After these immediate changes, intracranial pressure and perfusion gradually improve, although not back to baseline. A marked reduction in cerebral perfusion occurs near the hematoma and is probably due to local compression. Areas of the brain remote from the bleed also have alterations in perfusion, which are thought to be due to vasoconstriction caused either by chemical release from blood breakdown products or by neuronal mediation (diaschisis).
