1. Amoxicillin (Amoxil) 20-50 mg/kg/day PO divided tid [B] Easy dose: 30 mg/kg/day = 10 mg/kg/dose PO tid

Maximum: 250 mg/dose

Available in suspensions of 125 mg and 250 mg/5 mL

2. Cefuroxime axetil (Ceftin) 20-30 mg/kg/day PO divided bid [B] Easy dose: 20 mg/kg/day = 10 mg/kg/dose PO bid

Maximum: 250 mg/dose

Available in suspension of 125 mg/5 mL

3. Clarithromycin (Biaxin) 7.5 mg/kg/dose PO bid (for severe penicillin allergy) [C] Maximum: 500 mg/dose

Available in suspensions of 125 mg or 250 mg/5 mL Course: 21 days

B. STAGE II OR III—ARTHRITIC, NEUROLOGIC, CARDIAC SYMPTOMS

1. Ceftriaxone (Rocephin) 2 g IV qd for 21 days [B]

2. Consider doxycycline (Vibramycin) 100 mg PO bid [D] OR amoxicillin (Amoxil) 500 mg PO tid [B] in very mild cases Course: 30 days.
