(* Consider prophylaxis in high-risk patients.)

A. ADULTS AND CHILDREN OVER AGE 10

1. Amoxicillin (Amoxil) 2 g PO 1 h before procedure [B]

2. Clindamycin (Cleocin) 600 mg PO 1 h before procedure [B]

3. Cephalexin (Keflex) 2 g PO 1 h before procedure [B]

4. Azithromycin (Zithromax) 500 mg PO 1 h before procedure [B]

If unable to take PO

1. Ampicillin 2 g IV/IM 30 min before procedure [B]

2. Clindamycin (Cleocin) 600 mg IV 30 min before procedure [B]

3. Cefazolin (Ancef) 1 g IV/IM 30 min before procedure [B]

B. CHILDREN AGE 10 AND UNDER Note: Do not exceed adult dose

1. Amoxicillin (Amoxil) 50 mg/kg PO 1 h before procedure [B]

2. Clindamycin (Cleocin) 20 mg/kg PO 1 h before procedure [B]

3. Cephalexin (Keflex) 50 mg/kg PO 1 h before procedure [B]

4. Azithromycin (Zithromax) 15 mg/kg PO 1 h before procedure [B] If unable to take PO

1. Ampicillin 50 mg/kg IV/IM 30 min before procedure [B]
