Pharyngitis, infection of the pharynx and the tonsils, is a very common pediatric problem. It is estimated that $300 million are spent annually in its diagnosis and treatment.23 Despite physicians' long-standing familiarity with pharyngitis, there remains wide variability in approach. Controversies and new developments pertain to (1) selection of patients for throat culture and antibiotic treatment, (2) use of rapid diagnostic tests for group A b-hemolytic streptococcus (GABHS), (3) increased incidence of serious systemic streptococcal disease, and (4) occurrence of bacteriologic and clinical failure with penicillin treatment of GABHS.
