The Expected Home Death

Some infants are discharged for the NICU with lethal conditions for which further medical intervention is futile, and the parents are expecting the child to die at home. In many cases the parents are instructed to take the infant to the emergency department to be pronounced dead by a physician. The parents should be given a letter at the time of the original hospital discharge by their physicians delineating the infant's problems to provide guidance to the emergency physician facing such a situation.

This is a very traumatic time for the parents, and a futile resuscitation effort is not indicated and can prolong the parents' agony. However, it is very important to request autopsy permission to completely delineate the infant's problems and to provide optimal counseling for future pregnancies.
