Threatened Abortion

Threatened abortion is vaginal bleeding in the pregnant patient before 20 weeks gestation. It complicates about 25 percent of all pregnancies. Vaginal bleeding is usually light and is accompanied by crampy pelvic pain. On examination, abdominal, adnexal, and cervical motion tenderness is usually absent and the cervix is closed. The pivotal step in the workup of a patient with first-trimester vaginal bleeding is pelvic sonography. If a live IUP is seen and there is a single gestation, expectant management is the rule. Although limitation of activity is recommended, there is no proven therapy for early threatened abortion.
