The lower lip and mandible between the mental foramen and the midline, as well as the central incisor teeth, can be anesthetized by a mental nerve block. The nerve is the terminal branch of the inferior alveolar nerve. The position of the mental nerve is important to this block and varies with the age of the patient (9). In a young child, the foramen lies close to the inferior border of the mandible. In the adult, the foramen lies midway in the mandible. In an elderly patient, there is usually some atrophy of the mandible, and the foramen lies near the upper margin (Fig. 5). Usually, 1 ml of 1% lidocaine is injected at the root of the second bicuspid through an intraoral

injection. As mentioned, if difficulty is encountered in locating the foramen, remember that it is in line with the supraorbital and infraorbital foramina. It is important to aspirate periodically prior to injection of the anesthetic since both intravenous and intra-arterial injections are more likely in this region.
