Presence and Position of Firearm

Finding a firearm, particularly in proximity to the deceased, is supportive of suicide (59). In one Texas study, a firearm was found in the hand of a suicide victim, i.e., one finger in the trigger guard or at least one hand loosely gripping the barrel or grip, in 24 of all cases (94). If a handgun was used, it was found in the victim's hands in 25.7 of the cases. Of those using a long gun (shotgun, rifle), 19.5 had the firearm in their hand, usually the left. Another study showed 20 of victims with a handgun in their hand compared with 11 with long guns (95). A German study revealed that one-third of suicide victims had a firearm in their hands (21). A gun can be positioned in a victim's hand to conceal a homicide (94).

Other External Features of Contact Entry Wounds

Lifting of the skin against the barrel of the handgun, rifle, or shotgun can result in a muzzle imprint (Figs. 26 and 30 and ref. 16). It can be used to correlate a weapon with an injury, particularly if no bullet is recovered however, the muzzle imprint can be larger than the actual size on the weapon (16,113). The finding of a muzzle imprint also supports the circumstances of a multishot suicide (22,57,59,62). Variations of a muzzle imprint have been seen when a blank cartridge, nail gun, or ejector rod is involved (73,85,92,107).

Injury Potential and Lethality

The projectile directly crushes tissue (176). As a projectile passes through the body, tissue is temporarily (in the order of 10 ms) accelerated in a radial direction (16,176). The maximum radial extent of the temporary cavity is dependent on the amount of energy released from the projectile and the degree of tissue elasticity (16,142,176-178). The energy of a projectile increases exponentially based on its velocity (kinetic energy mv2 2 where m is the projectile's mass and v is the velocity 16,176-178 ). Civilian hunting rifles have velocities ranging from 600 to 1200 m s (2000-4000 ft s), civilian handguns less than 300 m s (1000 ft s), and a .22-caliber rifle 300 to 380 m s (1000-1250 ft s 177 ). Determination of Gunshot Residues

The detection of GSR can determine whether an individual fired a gun. GSR can escape from the muzzle end or breeches and blow onto the hand in contact or in close proximity to the firearm (159,215-218). Both hands should be tested (22). Long arms tend not to deposit as much residue on the hands as handguns (216). GSR can be on the palm or back of the firing hand or nonfiring hand used to steady the end of the muzzle (103,215,216). Primer residue can be transferred to the palm if a recently fired handgun is picked up and moved or the wound area is clutched (63,215,216). GSR can determine whether a hole on the skin and clothing is an entry (132,215). GSR can indicate firing distance. Residue (barium, antimony) has been observed on targets 3 to 4 ft (about 1 m) away depending on the type and caliber of pistol and revolver (219).

Penetrating Ballistic Injuries

Injuries caused by firearm missiles are classified as due to either low- or highvelocity missiles based on the missile's exit speed low-velocity rounds generally have a speed of 1000 ft s, whereas a high-velocity round is defined as one with a speed of 2000 ft s or above. The trauma caused is proportional to the kinetic energy of the missile and therefore proportional to the square of the velocity. Most handguns are considered low-velocity weapons. Low-energy ballistics inflict a smaller zone of injury, often limited to moderate soft tissue damage and minimally comminuted fractures. High-velocity missiles, however, inflict devastating, widespread destruction of the soft tissues, complex comminuted fractures, and organ damage. These injuries extend to distant areas of the body through an inconstant vector. Clinical Features

Patients with penetrating trauma to the flank should be resuscitated and evaluated according to standard resuscitative protocols. Initial resuscitative efforts should be directed towards the primary survey (see Chap.243). Information regarding the mechanism of injury, how much time has passed since the traumatic event, and the nature of the weapon should be obtained and recorded. In the first 12 cm (the average thickness of an adult human thigh) of a soft tissue wound path, there is often little or no difference between the wounding effect of low-and high-velocity bullets when the high-velocity bullet is of the military full metal jacket type. This is particularly true of the relatively heavier military-rifle bullets such as those fired by the AK-47 and NATO 7.62-mm (USA version) rifle. A wound of an extremity caused by an AK-47 bullet that does not hit bone is often similar to a handgun bullet wound. No matter how high its velocity, if a non-deforming, heavy bullet does not break, fragment, or hit a large bone, it may exit an extremity with much of its wounding potential unspent. These same bullets are often lethal in chest or abdominal wounds because the trunk is thicker than an extremity and allows the bullet a sufficiently long path through tissue to yaw. Other Scene Findings and Background Information in Suicide

The suicide victim may attempt to create a gun cleaning or hunting accident (see Heading 21 and refs. 16, 23, and 115). Careful investigation is needed to determine whether discharge of a gun under these circumstances was an accidental death (Fig. 4 ref. 44). Suspicions are aroused when handguns are involved in a hunting incident (42). A shotgun was the most common weapon in accidental deaths, either self-inflicted or inflicted on others (42,115).

Mechanisms Of Injury

Generally, penetrating injuries are classified as low-velocity or highvelocity injuries. Most stab injuries and handguns are low-velocity injuries due to the projectiles traveling at speeds less than 450 m s. Projectiles with speeds greater than 450 m s are considered to cause high-velocity injuries. Kinetic energy is proportional to the square of the projectile's velocity. Thus, higher-velocity projectiles cause much greater injuries, mostly due to temporary cavitation and secondary tissue destruction.2

Type of Firearm

There is a handgun predominance in American and most international studies (20-22,30,32,63-66). In the United States, this reflects the possession of firearms for protection. For example, in one jurisdiction in Texas, the following distribution of firearms was seen in suicide cases (32) Handgun Rifle Shotgun6 (23,34-37,46-49). Lower caliber weapons (handguns and rifles) are usual in multishot suicides, but higher caliber handguns and shotguns are used (67). Handguns using shotshell (pellets 68 ). Black powder percussion handgun (69).

Firearms

Rifles and handguns have rifled barrels designed to impart a spin to the bullet and allow a truer trajectory (16). Most shotgun barrels are smooth, and the usual projectiles are pellets. Rifled shotgun barrels can also fire large slugs (16). The pellets are either birdshot or larger buckshot, the diameter of the latter generally being greater than 0.25 in. (0.6 cm 16,124 ). If birdshot is used, the pellets can number in the hundreds (124). Caliber of a handgun or rifle refers to the approximate diameter of the barrel or base of the bullet (16). The gauge of a shotgun refers to the shotgun barrel diameter (124). A 12-gauge shotgun has a bore diameter that is equivalent to the diameter (0.729 in.) of 12 equally sized lead balls having a total weight of 1 lb (124). The exception is a .410-gauge shotgun, which has a bore of 0.410 in. (16). The only shotgun projectile that equals the bore diameter is a rifled slug (16). Gunshot Fractures

Handgun wounds of the extremities yield characteristic fracture patterns. Frequently seen are divot fractures of cortical bone, drill-hole fractures, butterfly fractures, In gunshot fractures from rifles and large handguns, a greater extent of comminution may be seen. These fractures often have complications because of the soft tissue damage these bullets cause.8 The vascular compromise associated with these comminuted gunshot fractures increases the likelihood of delayed union or nonunion of the fracture. Wound infections are more common in this group. Early fasciotomy to prevent compartment syndrome is important, when needed. At some hospitals, outpatient treatment is being used successfully for extremity fractures caused by handguns, if no significant neurologic or vascular compromise has occurred.21

Missile Embolization

Most bullets follow straight paths through the body, but sometimes, even in the absence of embolization, a bullet, particularly a handgun bullet, will not. It may ricochet off body structures, especially bone, or may follow fascial or tissue planes. Bullets traveling less than 1100 ft s (335 m s) are the ones most likely to be deflected by anatomic structures or to follow tissue planes. Bullet shape also influences the tendency to be deflected.

Laceration Tear

Since lacerations often result from contact with a foreign object, residue from this object may be left within the wound. Recognition and recovery of particles of concrete, brick, or sand may be very helpful in the determination of whether an injury resulted from impact with an object such as a rock or the butt of a handgun ( Fig. 257 6). This information should be included in the description of the injury.

Epidemiology

Accounted for nearly 11 percent of all murder victims in the United States in 1994, and nearly half of these children were between the ages of fifteen and seventeen. Among those killed in this age group, nearly 70 percent were killed with a handgun, while almost 20 percent were killed by another child. In addition, infants born to very young mothers have an increased risk of homicide (Overpeck, Brenner, Trumble, et al.).

Trauma Systems

The importance of a systems approach to trauma care becomes clear when one considers the timing of death occurring secondary to traumatic injuries. The pattern of mortality takes on roughly a trimodal distribution where three peak occurrences are seen. The first peak occurs in the prehospital setting largely due to devastating head and major vascular injuries. Efforts to reduce deaths in this setting are largely societal, complex, and multidisciplinary and include such multifaceted activities as drunk driving laws safe road construction seat belt, helmet, and airbag laws and violence-prevention activities such as counseling, education and outreach efforts, handgun control, and dissemination of conflict resolution skills. 45 A second peak incidence of deaths due to traumatic injuries occurs in the early minutes and hours after a patient's arrival at the hospital. The sharp bullet jacket edges that some soft-point and hollow-point handgun bullets have when they are deformed into a mushroom shape should be avoided.
