This is a systemic inflammatory disease with the characteristic pathologic finding of noncaseating granulomata. Neurosarcoidosis may affect the central nervous system and cause a wide range of neurologic manifestations. Periventricular abnormalities are seen on MRI in about one-third of patients with neurosarcoidosis; these may represent subependymal granulomas, areas of infarction secondary to granulo-matous angiopathy, hydrocephalic edema from basal meningeal involvement, or opportunistic infection such as PML.
