Helper-dependent adenoviral vectors possess many characteristics that make them attractive vectors for gene therapy of a wide variety of genetic and acquired diseases. However, acute toxicity provoked by the viral capsid currently hinders the clinical application of this otherwise promising technology. Clearly, studies to elucidate the mechanism(s) of Ad-mediated activation of the innate inflammatory response are needed. Perhaps with a clearer understanding of this phenomenon strategies can be developed to minimize, if not eliminate, this acute toxic response. If this can be accomplished, then HDAd should be able to provide sustained, high-level transgene expression with no further long-term toxicity.
