The various chlamydial infections show an analogous pattern of pathology. Initially, polymorphonuclear leukocytes infiltrate the locus of infection followed by lymphocytes, macrophages, and plasma cells. A strong Th1 response seems to be of major importance for the clearance of infection.
Recurrence of infection frequently occurs. This may be explained by the survival of chlamydiae inside the protected cell, possibly in the aforementioned so-called persistent state. Another possibly important factor is the ability of chlamydiae to suppress apoptosis.
