5-Fluorouracil and intralesional interferons as treatments for warts are more of historical interest. Four41-44 and six45-50 trials, respectively, were found for these treatments, most dating from the 1970s and 1980s. Evidence provided by all the trials was severely limited by heterogeneity of methodology and design and overall did not suggest any striking efficacy of either treatment.

Plantar Warts

Plantar warts are caused by the human papillomavirus. These warts are fairly common and contagious. They may be painful and are usually found over bony prominences. Single lesions are endophytic and hyperkeratotic. A mother-daughter wart is similar to a single lesion except for a small vesicular satellite lesion. Mosaic warts are often painless, closely grouped, and may coalesce. Diagnosis is usually made clinically. There are many therapeutic options. Treatment is complicated by the fact that many of these lesions will resorb spontaneously within 2 years. These patients may require prolonged treatment in resistant cases and should be referred.3,5

HUMAN PAPILLOMAVIRUS Human papillomavirus is probably the most common sexually transmitted disease today. HPV-6, -11, and -45 are most commonly associated with condyloma acuminatum, or venereal warts. Although most commonly affecting the anogenital region, oral lesions can occur. Intraorally, the labial and buccal mucosa, palate, tongue, and gingiva are most commonly affected, although any site can be involved. Lesions many be solitary or multiple and are sessile or pedunculated nodules. Treatment is with electrosurgical or laser therapy.1154

Four RCTs31-34 of different types of PDT reported varying success. Cure rates ranged from 8 (of patients) to 73 (of warts). One trial31 in 52 adults and children using a left-right design (randomising active and placebo treatments to warts on the left and right side of the body) showed resolution of warts in 40 of participants. In all those that responded to treatment, the warts on the placebo-treated side also resolved. In another trial in 40 adults,32 aminolaevulenic acid (ALA) PDT achieved a cure rate of 56 of warts compared with 42 of warts in the placebo PDT group. Topical SA was also used for all participants. One RCT34 in 28 adults with refractory warts compared four different types of light source for PDT with cryotherapy. PDT was administered three times and cryotherapy four times. The cure rates ranged from 28 to 73 of warts with the different types of PDT 20 of warts were cured with cryotherapy. Topical SA was also used for all patients.

Classical syndromes of chronic arsenic exposure include hyperkeratosis, corns, and warts on the feet (blackfoot) and hands. Studies from Taiwan and Japan strongly suggest that high arsenic intake is associated with cancers of the bladder, kidney, lung, and liver, which may be dose-related. Arsenic may be an indirect carcinogen.

The juvenile form is believed to be acquired during birth from maternal vaginal warts. The condition does not manifest, however, until months or years later when significant papillomata develop. In general, those who present early usually present with airway obstruction, while older children usually present with progressive hoarseness. Treatment is extremely difficult and frequent recurrence is a rule rather than exception. At laryngoscopy, there is usually extensive involvement of the whole larynx simultaneous tracheal and pharyngeal involvement by the papillomata is not uncommon. Repeated CO2 laser therapy offers the best control of the disease. In severe cases, antiviral therapy including interferon therapy may be tried. Although the effectiveness of treatment is unpredictable, spontaneous regression of the papilloma may occur at any age.

Pressure or irritation causes focal hyperkeratotic lesions of the skin of the foot. The cause of these lesions can be external (poorly fitted shoe) or internal (bunion). These areas of epidermal accumulation are defined as calluses. Calluses serve a protective function and should not be treated if they are not painful. Calluses grow outward but are soon pushed inward by continued pressure and become corns. Corns also develop in areas of scarring and between toes. Corns are classified as hard or soft. Hard corns are seen over bony protuberances where the skin is dry. Soft corns are seen between toes where the skin is moist. Corns may be painful or painless, but pressure on the corn usually produces pain. Corns interrupt the normal dermal lines and can thus be differentiated from calluses. Hard corns may resemble warts however, when pared, warts bleed and corns do not. Soft corns resemble tinea, which often leads to misdiagnosis and mistreatment. 23 and 4

Viral Cancers

Viruses are perhaps second only to tobacco as risk factors for human cancers. DNA tumor viruses include papillomaviruses and various herpes viruses (such as HHV-8, which causes Kaposi's sarcoma). More than sixty strains of human papillomaviruses (HPV) have been identified. HPV cause warts, which are benign tumors, but are also the causes of malignant penile, vulval, and cervical cancers. Infection with hepatitis B or C viruses is associated with increased incidence of liver cancer. Adenoviruses have been shown to induce cancers in animals, but not in humans. Retroviruses can also cause cancer in various animal species, including humans. HTLV-1 causes adult T-cell leukemia in about 1 percent of infected humans.

Malignant Melanoma

The incidence of malignant melanoma is increasing, thus making consideration of this disease process important. Malignant melanoma of the foot accounts for up to 15 percent of all cutaneous melanomas. Melanomas can present as an atypical, pigmented, or nonhealing lesion of the foot, including the nail. These malignancies often imitate more common foot disorders such as fungal infections and plantar warts. Since prognosis is directly related to early diagnosis, a high index of suspicion must be maintained. All skin lesions that are either atypical or not healing despite treatment should be referred for biopsy. 2948
